Agreement to Terms
By using our website www.luxdentallab.com ("Site") or submitting a prescription (Rx) either for physical or digital impressions/cases, whether signed or not, you agree to be bound by these Terms of Service and our Privacy Policy. If you do not agree to these Terms, you should not use our Site or submit prescriptions/orders to us.
Intellectual Property Rights
The content on the Site, including text, graphics, logos, and images, is the property of LUX Dental Lab, LLC and is protected by intellectual property laws. You may not use any content from our Site without our express written permission.
Prosthetic and Removable Restorations Guarantee
LUX Dental Lab, LLC offers a three-year guarantee on all prosthetic and removable restorations. This guarantee covers defects in materials and workmanship and is effective from the date of delivery. This guarantee is provided under the assumption that the products are used in accordance with standard dental practices.
Remakes
Remakes requiring a new impression will be charged at full price unless discussed otherwise in advance. Both the new impression and the original impression must be sent back to LUX; otherwise, a full-price charge will apply. If discrepancies are found between the new and original impressions, a full-price charge will also apply.
Shade Change
Shade changes to a darker shade will incur a shade change fee, reflecting the additional costs associated with darkening the shade. Conversely, changes to a lighter shade will be charged at full price, as this typically requires a complete remake of the product.
Rush Cases
Any request faster than our standard turnaround schedule is considered a rush case and will incur a rush charge.
Prescription (Rx) Submission
When you submit a prescription (Rx) to us, regardless of whether it is signed by the prescribing dentist, you represent that you have the authority to make such a submission and that all information provided is accurate and complete. LUX Dental Lab, LLC is not responsible for verifying the authority of individuals submitting prescriptions.
Payment Terms
A valid credit card must be kept on file as a condition of service unless alternative arrangements are approved by our management. We reserve the right to charge the credit card for any services rendered or products provided.
Refunds and Credits
Refunds or credits are issued at the discretion of LUX Dental Lab, LLC management and will be credited to the customer's account for future use. No cash refunds will be made unless specifically approved by management.
Chargebacks
Unauthorized chargebacks are not permitted and may result in termination of services or legal action. Formal written approval from LUX Dental Lab, LLC is required for any chargeback. Customers engaging in unauthorized chargebacks may be liable for legal costs incurred by LUX Dental Lab, LLC.
Governing Law
These Terms of Service shall be governed and construed in accordance with the laws of the State of Utah. All disputes arising under these Terms will be subject to the exclusive jurisdiction of the courts located within Utah.
Limitation of Liability
LUX Dental Lab, LLC will not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of the Site or from any prescription submitted, whether signed or not.
Changes to Terms
We reserve the right to modify these Terms of Service at any time. Your continued use of the Site or submission of prescriptions following any changes means that you accept and agree to the changes.
